If you need to apply for Italian citizenship through marriage, there is no Italian citizenship application form.
You will have to submit your application online instead, via the citizenship website of the Italian Ministry of Internal Affairs: cittadinanza.dlci.interno.it
The application process entails uploading a number of prescribed documents and filling in the required details.
The citizenship website is entirely Italian.
Unless you understand Italian or you are comfortable with translating the website as you go along, you might need some assistance.
